Medicaid Revalidation


To:                   Early Intervention Program (EIP) Agencies/Municipalities/Individuals with an Appendix Agreement

Date:               October 17, 2018

Purpose:         EIP Medicaid Revalidation

EIP providers including Department of Health Approved Agencies/Municipalities/Individuals with Appendix Agreements have been, or will be, notified by mail that they need to revalidate their Medicaid Enrollment with forms located at https://www.emedny.org/info/ProviderEnrollment/cmcm/Option2.aspx. It is important that the revalidation application and the required paperwork be complete when submitted to eMedNY, so that the Medicaid Provider ID is not terminated. Agencies and municipalities should ensure that their practitioners that order, refer, prescribe, or attend (OPRA providers), complete their revalidation when notified. Termination will result in ineligibility to receive reimbursement for provided services or to order/refer/prescribe/attend for all Medicaid beneficiaries, including Medicaid-eligible children receiving EIP evaluations or services.

NOTE: Agencies and municipalities must pay the application fee for their Revalidation when revalidating as EIP Providers. The application fee for Revalidation does not apply to individuals that are revalidating as EIP Providers.
